Acknowledgement of Vaccination Status

I, [Pet Owner Name], hereby acknowledge that my dog, [Dog's Name], has not received the vaccinations listed above. I understand that by choosing not to vaccinate my dog, there is an increased risk of my dog contracting contagious illnesses while at Jamie's Grooming and Blooming LLC.

Assumption of Risk & Release of Liability

I knowingly assume all risks associated with my dog being groomed without the above-listed vaccinations. I hereby release Jamie's Grooming, its owners, employees, and agents from any and all liability, claims, damages, or legal actions arising from or related to my dog contracting an illness during or after grooming services, regardless of whether the illness was contracted from another animal at Jamie's Grooming. I agree to hold Jamie's Grooming harmless for any veterinary expenses, damages, or losses associated with my dog contracting an illness.
